Disability Matters provides invaluable disability support services to the local community and is pro-active in advocating for the rights of people living with a disability & their families. We take a person centred approach to all supports, focussing on making sure a person with a disability is at the centre of all decisions and actions that relate to their life and their support.

NDIS PRE PLANNING ASSISTANCE TOGETHER

The The National Disability Insurance Scheme (NDIS) is a new way of providing support that takes a lifetime approach. Your first plan is the start of a lifelong relationship with the NDIS. That is why it is so important to take the time to pre-plan. Disability Matters are on hand to help make your journey as smooth as possible.

The NDIS can be confusing and daunting to navigate. We offer a free pre-planning service to help ensure you get the support you need included in your NDIS plan. Our pre-planning assistance will help to identify all the informal, community and mainstream services you already have and any reasonable and necessary supports you need to include in your NDIS plan to achieve your goals.

We will discuss your goals with you ahead of your conversation with the NDIS about joining the Scheme. You can clarify your thoughts and develop your ideas about what you want to achieve through your supports and services. We will answer all of your questions about the NDIS.

You will be informed, your plan will take less effort and you will have peace of mind knowing that you are ready to get the supports you want included in your plan when the NDIS rolls out in our region on 1 July 2018. Contact us now to arrange a time to talk.

SUPPORTED INDEPENDENT LIVING FOR YOU

Disability Matters assists clients living with a disability to live as independently as possible in the community, in their own home with compatible peers. Our support workers provide assistance with banking, shopping, domestic chores and skill development if and when required. Assistance and support is provided to enable the residents to pursue their own particular interests, and become contributing members of their community.

This support strengthens the client’s independence and gives peace of mind to their, often elderly, parent carers.

COMMUNITY PARTICIPATION ACTIVITIES FOR YOU

Including people with a disability in community life helps build more inclusive and diverse communities. It enriches community life for everyone and helps break down barriers and promote better understanding. Sometimes, people with a disability are prevented from participating in community activities because their needs are not taken into account, or because of negative attitudes of others.

We promote community inclusion by providing support to clients who desire to:

  • Maintain and develop life skills and increase independence
  • Continue learning and participate in meaningful leisure, recreational, social and cultural activities
  • Participate and be included in the local community
  • Have active and valued roles in the community
  • Expand friendships and support networks
  • Be supported in making the transition from school to adult life

SHORT STAY ACCOMMODATION FOR YOU

For those times when everyone just needs a break, we provide 24 hour support in accommodation at our Cook St Centre in Atherton. The Centre providers overnight care, 7 days per week. With up to four clients able to stay at a time, the Centre has built up a reputation of being a caring, supportive and enjoyable “home away from home” for people living with a disability.

Clients are encouraged and supported to participate in a variety of activities whilst at the Centre, both indoors and outdoors. Some activities will involve leaving the house to partake in excursions such as BBQ’s at one of many local lakes or parks. The Support team at the house also have access to a large selection of craft materials and other indoors activities.

Younger clients at the Centre often feel they have gained some independence away from their families whilst their parents have peace of mind knowing that their child is being appropriately supported.

IN HOME SUPPORT FOR YOU

Whether it’s personal care, skills training to develop independent living skills in and around the home or even help with household tasks, Disability Matters can help to provide personalised supports for you. We will help you achieve your goals in the best way we can.

CREATIVE GROUP ACTIVITIES FOR YOU

Our group activities can help you learn new skills, have fun and meet with others in similar situations. We are continually seeking new ideas for activities that will help our clients achieve their goals. Our activities are varied and include music and theatre groups, trivia days and ladies days to name a few. All are welcome and you don’t need any special skills to join in.

A band has been formed from our music group called “The Can Do Crew”. This group is very active and have written original songs with a goal of producing a CD. They are currently working on a musical play which they hope to present at the end of this year.

Our ladies days are a big hit with clients learning about makeup and skin care. It’s a great time for the girls to get together and be social.

There are all sorts of roles enjoyed by clients in these groups and it is wonderful to see the increase in confidence that they experience.

YOUR LIFE YOUR CHOICE (YLYC) FOR YOU

Disability Matters is a host provider for Your Life Your Choice (YLYC). This program is the precursor to the roll out of the National Disability Insurance Scheme, due to commence in our region on 1 July 2018.

Disability Matters offers you the opportunity to direct your supports and services to the extent to which you are confident and comfortable. We offer a range of options which can be utilised individually or together to build an individualised response for each person. You have the option to mix and match the type and level of assistance you want, and turn your package into the support you need.
